Case Management

The goals of the Case Management Program are to help individuals to live safely in their own homes; to help them find ways of financing needed services; and to put them in control of the services they feel they need in order to provide a continuum of care for as long as possible, delaying or preventing institutionalization.

Home and Community-Based Services (HCBS) have been demonstrated to be more cost effective than institutionalized care, and promotes a higher quality of life for individuals.

Case Management offers the following:

  1. An assessment of your current health and living situation.
  2. A meeting for your family and professionals to help you to develop an individualized plan of care, which can be adjusted if your situation changes.
  3. A professional case manager to answer your questions and make referrals to the agencies that provide those services.

You are assigned a case manager and some of the services you choose may have a cost involved. If needed, financial assistance may be available through a select funding source. For additional information click here for FAQ's.

Elderly Waiver:

Case Management Program works with the Dept of Human Services (DHS) in determining eligibility for the Elderly Waiver Program. To apply you must be at least 65 years of age or older, meet financial and level of care requirements. The level of care form is completed by your physician, to print a copy click here. The Medicaid T-19 Program is geared to help pay for home and community based services for those who qualify. To apply on-line for the Elderly Waiver/Medicaid Program click here www.oasis.iowa.gov.
